Supports Coordinator:

The position involves the location, coordination, and monitoring of needed services and supports for people with intellectual disabilities. Assists participants in obtaining and coordinating needed waiver and other services to ensure health and safety. The candidate must be organized, have excellent communication and  time management skills, the ability to work effectively with people with disabilities, to aid them to meet outcomes based on their needs. Caseload area; Montgomery/Philadelphia/ Chester County.  (note your preferred location)  This position requires travel to participants’ homes and community locations.

Bachelor’s degree from an accredited college or university which is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice or other related social sciences. Must present a clean PA Criminal background check, PA Child Abuse and FBI Criminal Record Check at time of hire.

ISP Reviewer:

The ISP Reviewer reviews, submits and tracks the Individual Support Plans for individuals supported in Supports Coordination.   The ISP Reviewer will review ISPs to ensure all pertinent information is in Individual Support Plans, will notify Supports Coordinators of needed revisions, and will track revisions to ensure ISPs are submitted according to ODP requirements.  This position reports directly to the Advocacy Executive Director.

The ISP Reviewer must present a clean PA Criminal Background Check, PA Child Abuse, and FBI Criminal Record Check.

Candidate must have a bachelor’s degree which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice or other related social science; OR two years’ experience in the social services field and two years of college level course work, which includes at least 12 college credits in courses listed above; OR any equivalent combination of experience and training which includes 12 college credits in courses listed above and one year experience in the social services field performing case management functions.  The candidate must also have been a Supports Coordinator for at least one year or relevant job experience as well as have knowledge in HCSIS. The ISP Reviewer will be required to maintain regular office hours and be available to the SCs for questions and needed follow up.
