Supports Coordinator:  Under the direction of the Supports Coordination Director.    Bachelor’s degree from an accredited college or university  which is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice or other related social sciences.  Must present a clean PA Criminal background check, PA Child Abuse and FBI Criminal Record Check at time of hire.   The position involves the location, coordination, and monitoring of needed services and supports for persons supported.  Assists participants in obtaining and coordinating needed waiver and other services.
